When they got back to the brownstone co-op on that last pre-Subsidized Thanksgiving was the first historical time Joelle intentionally did lines of cocaine to keep from sleeping. […] So Joelle was awake at 0400, cleaning back behind the refrigerator for the second time, when Orin cried out in the nightmare she’d somehow felt should have been hers.
Context
The scene's last sentence, eight years back: the night of the Headmaster's House Thanksgiving, Joelle wired on her first deliberate wakefulness-cocaine while Orin sleeps. No dreamt content is rendered — what's rendered is dream-envy, a nightmare as family property landing on the wrong sleeper. The dental nightmare that opens the scene is arguably the one she finally collects; cf. Orin's Moms-head serial (012) for what he was plausibly crying out about.
